1. Read the required Concussion Resources on the Ontario website.

2. Read the Wyoming Ball Hockey League Concussion Code of Conduct.

3. Download the Registration Form.

3. Email the completed registration form to wyomingballhockey@gmail.com

4. Pay the $40 or $30 registration fee by email transfer.

• Include player's name in message

5.  You will receive registration confirmation within a couple of days.
